| In December 2016, Oxy2go notified us they
were no longer going to market their Oxy2go System II. At that time
we offered to continue offering the Oxy2go System II product to dealers
that qualify for wholesale accounts. To open a wholesale account
contact OPB Entertainment, aka Oxygen Party Bar. You must have an
established business and be marketing related products; i.e. oxygen,
health care, recreational, etc. If you are a California enterprise,
you must also have a CA Sellers Permit and submit this form (BOE)
